I've got Content Protect. It has all of the features that you mentioned, but it can drive you crazy with the password overrides. I'm not sure exactly how the filter works, but some very benign sites get blocked as Adult Content.

I know several people that use Net Nanny, which is fairly similar to Content Protect.
